GLP-1 medications such as Wegovy, Ozempic, Mounjaro and Saxenda are injectable versions of this hormone that keep those signals switched on for longer. The result: reduced appetite, fewer cravings, smaller portions that feel satisfying — and, paired with a sustainable lifestyle, clinical studies show patients lose up to 20% of their body weight.

How does GLP-1 actually work inside the body?

GLP-1 works in three places at once:

Is GLP-1 safe for long-term weight management?

GLP-1 medications have been prescribed for type-2 diabetes for over 15 years and for obesity since 2014, with extensive long-term safety data.

The most common side effects (nausea, fatigue, constipation) are mild and usually resolve within four to six weeks. Serious side effects are rare but possible — which is why GLP-1 is prescription-only and must be supervised by a licensed physician. A proper metabolic assessment screens for the conditions where GLP-1 is not appropriate.

Who qualifies for GLP-1 treatment?

In most health systems, GLP-1 for weight management is prescribed to adults with a BMI of 27 or higher plus a weight-related condition (such as high blood pressure, pre-diabetes, sleep apnoea or high cholesterol), or a BMI of 30 or higher regardless of other conditions.

A licensed physician makes the final decision after a full medical history, current medications review, and lab work. Pregnancy, personal or family history of certain thyroid cancers, and some pancreatic conditions are contraindications.

What is the difference between Ozempic and Wegovy?

Ozempic and Wegovy are the same active ingredient — semaglutide — made by the same manufacturer (Novo Nordisk). The difference is the approved use and the dose.

Ozempic is licensed for type-2 diabetes at weekly doses up to 2.0mg. Wegovy is licensed specifically for weight management at weekly doses up to 2.4mg. Clinically, Wegovy produces slightly more weight loss on average because of the higher ceiling dose.

If you are starting GLP-1 purely for weight management, Wegovy is usually the first choice where it is available.

Mounjaro vs Wegovy — which one is better?

Mounjaro (tirzepatide) and Wegovy (semaglutide) work in related but different ways. Wegovy activates the GLP-1 receptor only. Mounjaro activates GLP-1 plus a second gut-hormone receptor called GIP — which is why it is called a dual agonist.

In head-to-head clinical data, Mounjaro produced about 22% average weight loss at the highest dose vs about 15% for Wegovy. That said, Mounjaro is newer, usually more expensive, and not always available. A physician will match the medication to your history, tolerance and goals — better does not always mean right for you.

Semaglutide vs tirzepatide — which is safer?

Both semaglutide (Ozempic/Wegovy) and tirzepatide (Mounjaro/Zepbound) have excellent long-term safety profiles when prescribed correctly. Side-effect rates are broadly similar — mild nausea, fatigue and digestive upset early on, usually resolving within four to six weeks.

Tirzepatide has a slightly higher reported rate of early-week nausea because of its dual action, but the trade-off is typically faster weight loss. The right choice depends on your metabolic profile, appetite pattern, previous GLP-1 experience, and price considerations — which is what a proper consult is for.

Is Saxenda still used, or is it outdated?

Saxenda (liraglutide) is the original daily GLP-1 for weight management. It is not outdated — it is simply older.

It's now usually considered a second-line option because it requires a daily injection instead of weekly, and average weight loss is lower (around 6–8%) than Wegovy or Mounjaro. Saxenda is still prescribed when the newer options are unavailable, when a patient does not tolerate semaglutide, or in specific clinical circumstances. It has over a decade of safety data behind it.

Which GLP-1 medication is right for me?

There is no universal answer. The right medication depends on your current health, previous weight-loss attempts, tolerance to side effects, goals, and what is available and affordable in your country.

What should I expect in the first month on GLP-1?

The first month is almost always the hardest and the most rewarding.

In week one, most patients notice appetite suppression and the quieting of food noise within 48 to 72 hours of the first dose. Mild nausea is common, especially after larger meals. By weeks two and three, the nausea usually settles, hunger stays low, and 2–4 kg of weight loss is typical. Week four often brings a small plateau as the body recalibrates — this is normal, not a failure.

Consistent hydration, high-protein meals and light daily movement make the first month dramatically easier.

What are the GLP-1 side effects, week by week?

Week 1: mild nausea and reduced appetite, occasional fatigue; most common in the first 48 hours after each injection.

Week 2: nausea typically improves; constipation or loose stools may appear.

Week 3: most digestive side effects resolve; you may notice food tastes stronger.

Why did my weight loss plateau at week four on GLP-1?

The week-four plateau is one of the most common and most misunderstood moments on GLP-1. It happens because your body adjusts water balance and metabolic rate to the new lower weight, and the initial rapid loss settles into a slower, more sustainable pattern. It does not mean the medication has stopped working.

Most patients break the plateau within 7–14 days without changing the dose — especially when they increase protein, drink more water, and add gentle resistance training. A plateau beyond three weeks usually triggers a scheduled dose titration with your doctor.

How long do I need to stay on GLP-1?

Weight management is a long-term condition, and most clinical guidelines now treat GLP-1 as a long-term medication — similar to how we treat high blood pressure.

Patients typically stay on a full dose for 12 to 18 months to reach their goal, then work with their physician on a slow, structured taper. Some stay on a maintenance dose long-term. Stopping abruptly almost always causes partial weight regain — this is expected biology, not willpower failure. A good programme plans the taper from day one.

What happens to my weight after I stop GLP-1?

Without structured support, published data shows patients regain roughly two-thirds of lost weight within one year of stopping.
